We understand and respect the magnitude of the decision to move a loved one into a community such as Country Manor Health Care Center.

We do not take this responsibility lightly. 

Our award-winning culture is at the heart of everything we do. We strive to make every resident feel special, important, and cared for each and every day. 
WHAT IS HOMETOWN CULTURE?

Hometown Culture is an original and innovative model of care created by Country Manor Health Care Center. 

Upon arrival, residents and their loved ones are greeted by a personal care team that is devoted to learning about individual personalities and preferences. This includes everything from favorite music and lifelong interests to comfort foods and personal routines. 
Personal care teams may be made up of staff members from the following areas: nursing, culinary, housekeeping/laundry, Champion Rehab, Social Services, Therapeutic Recreation and Spiritual Care (by request). 

The goal of the team is to alleviate the initial stressors of moving, and to provide on-going companionship, and support. The team helps create an environment that is encouraging and comforting for each distinct personality.

If a resident expresses their love for animals, we will make sure they receive frequent pet visits from our Resident Therapy pug, Sophie. If another resident tells us of a history in education, we will arrange for Kids Country students to visit and help facilitate educational activities. Examples like these happen all day, every day at Country Manor Health Care Center. 
We work incredibly hard to create special moments 7 days a week for the people we serve.
